As for 1) and 2), can you tell us what YOU think would be best?
For 1), that format-specific media types be used. For 2), I lean towards not using a suffix, but won't argue vociferously against it should the WG want it.
As for 3), please note that there is no need for a format to be (or expected to going to be) popular in order to register a media subtype.
